Catholic Health Cancer Institute – Patient Navigation Program
The patient navigation program is made up of caring, trained professionals to guide patients through barriers of care within a complex health system, to help ensure timely diagnosis and treatment.
Our Patient Navigation Program is designed to support you through every stage of your journey from your treatment plan, coordination of care, to recovery. Our Catholic Health Cancer Institutes have nurse navigators that also assist with support services for you and your caregivers.
Our Patient Navigators offer knowledgeable and comprehensive support to patients and their families, including:
- Guidance through the health care system, providing health resources and support services.
- Coordinating appointments, testing, treatments and follow-up services.
- Identifying and overcoming barriers to accessing and completing the care you need.
- Providing compassionate, personalized support to help reduce anxiety and stress.
- Fostering communications between the patient and the health care team, including physicians, genetic counselors, nutritionists, social workers and others.
- Educating you on the treatment therapy you need to help you make informed decisions and better understand your condition and manage symptoms and side effects.
- Connecting you with support services, community support and other services (e.g., social work, pastoral care, nutrition, financial counseling).
- Working with you and your care team to develop a survivorship care plan.
