About Catholic Health MyChart
Catholic Health MyChart offers personalized and secure online access to your medical records. It enables you to manage and receive information about your health. With Catholic Health MyChart, you can:
- Schedule medical appointments.
- View your health information, including medications, allergies, test results, and more.
- Request medication refills.
- Access resources for trusted health information.
- Message your care team.
No, Catholic Health MyChart is a free service for all patients.
You need access to a computer or mobile device connected to the Internet and an up-to-date browser (such as Edge, Chrome, Firefox, or Safari). You can also use the Catholic Health MyChart mobile app on a mobile device.
We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account must be accessed using that password. On mobile, the account may also be accessed using face ID or fingerprint login. Unlike conventional email, all Catholic Health MyChart messaging is done while you are securely logged in to our website or mobile app.
You can ensure that your account stays secure even if someone else has your username or password by turning on two-step verification. When this feature is turned on, you must enter a code that is sent to you by email or text message to log in to Catholic Health MyChart, in addition to using your username and password.
Enrollment Questions
There are several different methods of Catholic Health MyChart signup that might be used by different departments across your healthcare organization:
- Clinic staff might sign you up directly while you're at the front desk or in the exam room.
- You might receive a Catholic Health MyChart activation code on your After Visit Summary or on a billing statement.
- You might receive a text message or email with an activation code when you come in for a visit.
- You might be able to use self-signup online to create a Catholic Health MyChart account by matching your information against what is on file in your medical record.
If you don't have an activation code, you can request one on the sign-up page. After we verify your information, you will receive a new code.
For your security, your activation code expires after a set period of time and is no longer valid after the first time you use it. If you still have problems, call our Catholic Health MyChart Patient Support Line at 631-465-6100.
Paperless Billing
As of November 5, 2024, all Catholic Health MyChart users are automatically enrolled in paperless billing. With paperless billing, you receive email or text message notifications about new bills and can make payments in Catholic Health MyChart or the Catholic Health app. You will not receive paper statements in the mail for most of your bills from us.
To manage your paperless preferences:
1. Log in to your MyChart account on our website or the Catholic Health app.
2. Go to the Menu on the upper left, and scroll down to Billing.
3. Select Manage Paperless Billing.
4. Select the toggle on the right side of the page to change your paperless billing status, then select “Save changes” at the bottom of the page.
You must have an email or mobile phone number on file to receive paperless bills, so we can send you notifications about new bills. To get text message notifications, you must opt in to receive billing text messages from Catholic Health. Visit the Communication Preferences page in MyChart to manage your text message subscriptions.
Your Medical Record
With Catholic Health MyChart, you can view most test results as soon as they become available. Know that you may likely see results before your healthcare provider has had a chance to review them. After your provider reviews your results, you might see additional information in Catholic Health MyChart.
You will generally receive a response within 2-3 business days. Note that Catholic Health MyChart messages should not be used for urgent situations. Please seek treatment at your nearest emergency room if the situation requires immediate attention and/or dial 911 if it is an emergency.
On the Personal Information page in Catholic Health MyChart, you can update your address, phone number, email address, preferred name, and other personal details at any time so that your clinic always has the most up-to-date information in your record.

Your Catholic Health MyChart information comes directly from your electronic medical record. If you feel that some information may be incorrect, you have the right to request an amendment (or correction) to your health information. Technical Questions
If you're having trouble logging into your MyChart account, click the "Forgot login information?” link for assistance. You can either use CLEAR to get your username or reset your password, or you can receive a code to your email or cell phone on file. You can also contact our Catholic Health MyChart HelpDesk at 631-465-6100 to request assistance.
Catholic Health is committed to protecting your personal and private information. As part of this commitment, we have partnered with CLEAR, the identity verification company you may recognize from airports and stadiums.
You can use your existing CLEAR account or create a new, free account to verify your identity to recover your MyChart username and password or to create a new MyChart account.
This is the process for setting up your free CLEAR account with Catholic Health:
- You will be directed from Catholic Health MyChart to the CLEAR website.
- Before collecting your information, CLEAR will ask for your consent.
- Enter your mobile phone number. You cannot create a CLEAR account without a mobile phone number that can receive text messages.
- Enter your email. This email must be unique to your CLEAR account and cannot be shared with another CLEAR account.
- Enter the code sent to your contact information.
- Take a “selfie.” You cannot wear glasses or a hat in this photo unless your government ID has them. You will also need to have enough lighting and be looking straight at the camera.
- Take a photo of your government-issued ID. You will choose the country of the ID and the type of ID before you take the picture.
- After you successfully follow these steps, you will see a thank you message and then be sent back to the Catholic Health MyChart website.
- If we were able to match your contact information in our records with your CLEAR account information, we will guide you through account recovery or creation process. If we can’t find a matching account, we will direct you to another process to recover your account information or create an account.
If there are errors in any of the above steps, you will get an error message on the CLEAR website with instructions to follow or a link to contact their support team.
Please note, this CLEAR account is not the same as “CLEAR Plus,” so you will not be able to use your free CLEAR account to move faster through airport security lines.

First, try checking your spam or junk folders. If you did not receive the email or text with your code, try clicking Resend Code. If you still haven't received the code, it could be that your account has a different email address or phone number on file.
If this is the case, call our Catholic Health MyChart HelpDesk at 631-465-6100 for support. However, the MyChart HelpDesk cannot update the contact information on your account. You will need to update your information in person at your next visit to Catholic Health.
To reduce risks to your privacy, if you remain idle for 10 minutes or more after you log in to Catholic Health MyChart, you will be automatically logged out. In general, we recommend that you log out of Catholic Health MyChart if you need to leave your computer or device for any period of time.
After 5 unsuccessful attempts to log in, you will automatically be redirected to the account recovery page. If you are unable to recover your account, please contact the MyChart HelpDesk at 631-465-6100 for assistance.
Please note, after 10 unsuccessful attempts, you will be locked out of your account for 30 minutes.
If you've been seen at another healthcare organization or office that uses Epic for their electronic health record, you might be able to view information from that organization’s medical record right in your Catholic Health MyChart account. You might have heard of this feature called Happy Together.
This includes information from other organizations, such as:
- allergies
- care team
- health issues
- medications
- messages
- test results
- visits/appointments
To view this information across organizations, you must “link” your account. Watch our video tutorial on how to do this.

Sharing my Health Information via Catholic Health MyChart
Yes, you can, if you choose to and allow such. This is called Proxy Access and allows a parent (or guardian), family member, caregiver or other personal representative of your choice, to log into his or her own personal Catholic Health MyChart account, and then connect to your health information.
If you are an adult and are requesting proxy access be given to another adult, or if you are a parent/legal guardian and requesting proxy access to a child/minor’s or teen’s information via Catholic Health MyChart please take a moment to review this very important information before taking any further steps.
*Please note, for the purposes of access to health information, children/minors between the ages of 12-17 (“teens”) who are consenting to medical treatment, as permitted under NYS law, may affect and limit a proxy’s level of access in Catholic Health MyChart This is because of regulations protecting a teen patient’s ability to seek confidential health care in some situations.
If you are more comfortable, a representative at a Catholic Health hospital or health care provider’s office may assist you or your selected proxy in providing access to your health information via Catholic Health MyChart, in person.
