Your Final Step Home: The Discharge Concierge Center

As part of your hospital discharge, your care team may invite you to spend a short time in our Discharge Concierge Center. This is a comfortable, welcoming space designed to make the last step of your hospital stay smooth, relaxed, and stress-free.

The Discharge Concierge Center is available for patients who qualify.  These patients may be picked up outside of the Emergency Department patient drop off entrance.  All other discharge patients will be picked up from the Main Entrance of St. Francis Hospital.

Once your care team has completed your discharge, moving to the Discharge Concierge Center allows you to:

  • Complete your discharge in a calm, non-clinical setting
  • Wait comfortably for your ride home

This is a normal and expected part of the discharge process for many patients.

What to Expect in the Discharge Concierge Center

  • Comfortable seating in a quiet, welcoming environment
  • Coffee, beverages, and light snacks available as needed
  • If you participate in Catholic Health’s Meds to Beds program, your medications will be delivered directly to you in the Discharge Concierge Center.
  • Your ride will arrive at the door just outside the Center.  Upon arrival, they will notify staff, by calling 516-562-6622.
  • When your ride home arrives at the hospital, a team member will escort you safely to your vehicle.

Our staff remains available to support you during this final step of your stay.

Your care team will let you know if the Discharge Concierge Center is included in your discharge plan.

Transitioning to the Discharge Concierge Center means:

  • Your hospital care is complete
  • You are ready to go home
  • Your final steps of hospital care are being handled with comfort, care, and attention

Family FAQs

Transportation arrives outside the Discharge Concierge Center. When you arrive, simply call the number provided by staff, and a team member will escort the patient safely to your vehicle.

Patients typically stay briefly while waiting for their ride. The Center is designed for short, comfortable transitions home.

Yes. Coffee, beverages, and light snacks are available. If a light meal is needed, staff will assist as appropriate.

If the patient is enrolled in Catholic Health’s Meds to Beds program, their medications will be delivered directly to the Discharge Concierge Center before departure.
