Comforting Care at Home
Home Care services provide patients in Suffolk, Nassau and Queens Counties with the support they need to recover from a recent illness or surgery after returning home from the hospital. Care is directed by a physician and provided by a team of specialists with a wide range of expertise and training.
Your First Home Care Visit
Once you return home, your nurse or therapist will call to schedule a time for your first home care visit. The initial appointment typically takes about two hours to complete. It includes:
- A review of your immediate living area to offer suggestions and education about ways to improve your safety at home.
- Evaluation and assessment of your current health to further determine your care needs.
- Identification of your physical strengths and weaknesses to develop a starting point from which your progress is measured.
- Instruction on fall prevention and emergency preparedness
- Review of all your medications, including over-the-counter medication and dietary requirements.
Home Care Services Offered:
- Disease and Pain Management: Including stroke, diabetes, COPD, cardiac disease, joint care and recovery from surgery.
- Skilled Nursing: Expert RNs and LPNs will provide skilled nursing care, including treatments, education, medication review and safety evaluations.
- Physical Therapy: Evaluation and strength training, transfer training, home exercise programs, post‑surgical rehabilitation and other therapeutic treatments.
- Occupational Therapy: Services that enable patients to regain independence in performing activities of daily living and personal care.
- Speech Language Pathology: A progressive rehabilitation program that helps to restore the vital functions of speech, swallowing and communication.
- Medical Social Services: Social Workers provide information and referrals to community resources, assistance with nursing home or assisted living resources, help in applying for Medicaid or other entitlement programs, and counseling for long term planning.
- Home Health Aides: Intermittent personal care, assistance with bathing, dressing, meal preparation and home exercise programs under the direct supervision of the RN or PT.
- Nutrition Counseling: Registered Dieticians provide nutritional counseling, as needed.
- Medication Management: Monitor and manage medication regimes to ensure compliance with physician orders.
- Telehealth: Daily readings and monitoring of patient’s heart rate, blood pressure, weight and oxygen saturation are transmitted via a home monitoring system.
- Spiritual Care Services: Chaplains offer spiritual and emotional support, addressing various patient issues.
- Behavioral Health: Provides the expertise of behavioral health nursing and community resources, including those affected by depression and anxiety related to their medical condition.
- Infusion Services: A team of specially-trained clinicians provide care, education and support for those receiving infusion therapy.
- Fall Prevention and Balance Treatment: Coordination of care with your physician to provide ongoing assessment, education and treatment to reduce fall risk.
- Pediatric Care: Education on symptoms and management of pediatric illnesses, including asthma, diabetes and cystic fibrosis.
- Wound Care: Evaluation and management of wounds and ostomies.
Frequently Asked Questions
If you are covered under Medicare part A benefits, Catholic Health Home Care services may be available to you at no cost. We accept most insurance plans including Medicaid.
You are responsible for full payment of any and all co-payments, deductibles and services that are not covered by your insurance plan or financial assistance resources. If you have financial difficulties related to your home health care needs, our social workers can help identify resources that offer assistance.
We contact you before home care visits discuss the frequency and timing of visits.
Catholic Health’s Home Care program operates 24 hours a day, seven days a week, including holidays. We always have a nurse on call to answer your questions and assist with any care issues you may have.
